Insurance

I am on not on any insurance panels, but I will work with you to help you get out-of-network coverage from your insurance company.  I will ask you to pay me upfront, and then I will help you submit the information to your insurance company so that you can request reimbursement from them. 

Payment I accept cash, check and all major credit cards for payment.

Cancellation Policy I have a 24-hour cancellation policy.  This means that if you need to miss an appointment, it is important for you to let me know at least 24 hours before your scheduled time with me.  I will charge a full fee for missed appointments that are not cancelled within that time frame. 

For EFT Training

Please go to the page on my website that describes the training you are interested in (either Four Day Externship or Core Skills Training) and scroll to the bottom of that page where you will find a registration form.  You can register and pay using that form.
